ABSTRACT

Abstract Background: Sexually transmitted infections are a global public health problem. Routine analysis includes microbiological and serological tests for the diagnosis of pathogens. Atypical microorganisms such as Chlamydia trachomatis and mycoplasmas are not determined due to the requirements for their identification. Furthermore, Gardnerella vaginalis is not included despite being associated with bacterial vaginosis. Objective: To develop a multiplex PCR to diagnose Chlamydia, mycoplasmas, and Gardnerella. Method: Standardization of multiplex PCR tests was carried out using oligonucleotides for the identification of Chlamydia (ompA gene, plasmid orf6), Mycoplasma/Ureaplasma and Gardnerella (rRNA16s genes). Results: Multiplex PCR tests were standardized for the microorganisms studied, optimizing the concentrations and conditions of the multiplex reactions. Duplex PCR was obtained for Chlamydia (ompA, orf6), Chlamydia/Gardnerella, and Chlamydia/mycoplasmas, and triplex PCR for Chlamydia/mycoplasmas. Also, a quadruplex for Chlamydia, Mycoplasma/Ureaplasma and Gardnerella. PCR and automatic hybridization verified the results obtained (HybriSpot 12) and in silico analysis. Conclusion: Multiplex PCR tests with high sensitivity and specificity were developed to identify C. trachomatis, mycoplasmas, and G. vaginalis.

ABSTRACT

Introduction: Maternal and child health are essential to public health, especially during pregnancy, where urogenital infections can affect mothers and fetuses. Sexually transmitted infections (STIs) increase obstetric risks and have complex connections with the human immunodeficiency virus HIV. In Brazil, pregnant women with HIV are a growing concern, requiring focus and appropriate interventions. Objective: This study aimed to examine the clinical and epidemiological characteristics of urogenital infections in pregnant women with and without HIV and to assess whether there are notable differences between these groups. Methods: A scoping review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews (PRISMA-ScR) and Joanna Briggs Institute guidelines. Databases such as Medical Literature Analysis and Retrieval System Online (MEDLINE), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), and Scientific Electronic Library Online (SciELO) were explored using relevant terms. Inclusion/exclusion criteria selected nine studies for analysis. A Population, Intervention, Comparison, Outcome, and Study Design (PICOS) approach directed the search. Results: Pregnant women with HIV had a high prevalence of STIs, including Chlamydia trachomatis, Neisseria gonorrhoeae, Trichomonas vaginalis, and bacterial vaginosis. HIV infection appears to influence the risk and severity of urogenital infections. Pregnancy increases the risk of STIs, regardless of HIV status. Male partners may also influence the presence of STIs in pregnant women, especially those with HIV. Conclusion: This study highlights the association between HIV status and urogenital infections in pregnant women, indicating the need for appropriate screening and care. Prevention and treatment of STIs in pregnant women are essential for maternal and child health, regardless of HIV status. An in-depth understanding of these issues can improve public policies, clinical practices, and preventive interventions that target the overall health of these vulnerable populations. Keywords: HIV. Métodos: Uma revisão de escopo foi conduzida, seguindo as diretrizes Preferred Reporting Items for Systematic Reviews and Meta-Analyses - Extension for Scoping Reviews(PRISMA-ScR) e Joanna Briggs Institute.

ABSTRACT

ObjectiveTo study the sexually transmitted infections (STIs) in pregnant women with non-local household registration in Xuhui District and analyze the risk factors for STIs. MethodsFrom April 2020 to March 2022, pregnant women with non-local household registration who received their first prenatal examination in a general hospital in Shanghai Xuhui District were selected to conduct a status survey of STIs. Logistic regression model was used to analyze the influencing factors of bacterial vaginitis and ureaplasma uaplasma infection. ResultsThe top three infection rates in the pregnant women were Ureaplasma urealyticum (13.2%), bacterial vaginosis (9.8%) and mycotic vaginitis (4.7%). Age between 25 and 35 years (aOR=0.53,95%CI: 0.28‒0.98) and monthly income ≥8 000 yuan (aOR=0.30,95%CI: 0.11‒0.82) were significantly correlated with ureaplasma uaplasma infection. Pregnancy number of 2 (aOR=4.95, 95%CI: 1.59‒15.46), first sexual relationship occurred before marriage (aOR=2.83, 95%CI: 1.04‒7.74), husband's alcoholism (aOR=3.83, 95%CI: 1.08‒13.56), high school education (aOR=0.27, 95%CI: 0.08‒0.93), and husband's travel history (aOR=0.30, 95%CI: 0.12‒0.79) were significantly correlated with bacterial vaginitis. ConclusionPregnant women with more gestation times, first sexual intercourse before marriage and husband with heavy drinking are more likely to be infected with bacterial vaginosis. Pregnant women with younger age and lower income are more likely to be infected with Ureaplasma urealyticum. Follow-up and monitoring should be strengthened in these groups.

ABSTRACT

Chlamydia trachomatis (CT) is the most common curable sexually transmitted non-viral infection, which can cause urethritis, prostatitis, orchitis and epididymitis among males. Toxoplasma gondii, a protozoan causes toxoplasmosis among humans. These infections may lead to decreased fertility. Hence, this study was aimed to estimate the seropositivity of CT and T. gondii infection among the male partners of infertile couples in Odisha, India. In this facility-based cross-sectional study, first void urine, seminal fluid and blood sample were collected from 153 males attending infertility clinics including 74 healthy controls. Urine and seminal fluid were tested through polymerase chain reaction (PCR), and enzyme-linked immunosorbent assay was used to detect immunoglobulin (Ig) G, IgA and IgM anti-Chlamydia and IgM anti-Toxoplasma antibodies through serum. The prevalence of CT was 1.26 per cent (95% CI: 0.03-6.85) as estimated from semen samples using PCR. We detected anti-Chlamydia antibodies IgM-four per cent [95% confidence interval (CI): 0.83-11.24]; IgA-28.16 per cent (95% CI: 18.13-40.09) and IgG-12.5 per cent (95% CI: 5.87-22.4) among participants. Anti-Toxoplasma antibodies IgM were observed in 27.63 per cent (95% CI: 17.98-39.08) of participants. None of the control samples were found positive. Overall seropositivity of CT and Toxoplasma infections is comparable, which suggests that greater attention is required for screening these infections at clinics, especially among infertile couples.

ABSTRACT

Abstract Lymphogranuloma venereum (LGV) is a sexually transmitted infection caused by Chlamydia trachomatis bacteria. In the past years, it has emerged as a relevant infectious agent, mainly affecting men who have sex with men (MSM), frequently associated with other sexually transmitted infections as human immunodeficiency virus (HIV) and syphilis. Even though genital lesions correspond to the classical presentation of LGV, nowadays anorectal syndrome is more frequently reported. We present a MSM patient, HIV infected, being treated with antiretroviral drugs, with a history of recurrent genital herpes, also recently treated for a syphilis in a latent stage. He had no recent trips. He referred inguinal fistulized enlarged lymph nodes, associated with a painful genital ulcer, he denied anorectal or systemic symptoms. He had been treated with multiple antibiotic and antiviral drugs, with no clinical response. Molecular studies from the content of the fistulized lymph node identified C. trachomatis. The patient received doxycycline for 6 weeks, showing good clinical response.

ABSTRACT

Abstract Neisseria meningitidis is a Gram-negative bacterium frequently associated with invasive diseases with high mortality. Although its natural reservoir is the human nasopharynx, in recent years there have been increasing reports of isolation of this agent in the anorectal mucosa, mainly in men who have sex with men (MSM). We present the case of an HIV-positive MSM who consulted for urethritis and primary syphilis, in which N. meningitidis was isolated in an anorectal specimen. He was treated empirically with ceftriaxone and azithromycin, and a post-treatment control culture was negative. Despite the increase in anogenital infections and colonization by N. meningitidis, its role is unknown as a genital pathogen and in the transmission of other infections and the need for specific therapeutic regimens.

ABSTRACT

The illness can reveals impacts and consequences of social, economic and cultural inequalities. To evaluate the seroprevalence of HIV infection, Hepatitis B and C and Syphilis and sexual behavior of residents of the Nova Amazônia Settlement Project (PANA). Observational, cross-sectional, descriptive, quantitative and qualitative study, involving 246 residents of PANA, carried out between November 2019 and September 2020. A questionnaire was applied and rapid tests were performed for HIV 1 and 2, Syphilis and Hepatitis B and C. A prevalence of 4.1% for sexually transmitted diseases was observed, statistically associated with being male (p = 0.04), being over 40 years old (p = 0.003) and having been related with more than 10 sexual partners throughout life (p = 0.03). Due to the identification of risky sexual behaviors, such as very low adherence to condom use, it is necessary to perform an early diagnosis and monitor new cases.

ABSTRACT

Abstract INTRODUCTION: Lipschütz ulcers are an infrequent cause of genital ulcer, of spontaneous relief and almost always benign, although in some patients they may leave genital scars. They usually appear in young women, before the onset of active sexual life, coinciding with a catarrhal or flu-like condition. CLINICAL CASE: 18-year-old female patient with a painful vulvar ulcer, coinciding with an episode of acute tonsillitis and fever of 38 ºC. The patient denied having had sexual intercourse. The ulcer was 3 to 4 cm, deep, purulent, with opening of the right labium minora and with a significant necrotic component. For pain control she was prescribed corticosteroids, oral doxycycline, lidocaine cream and anti-inflammatory drugs. Laboratory tests ruled out sexually transmitted infection. At the end of the prescribed therapeutic scheme the evolution was favorable, with disappearance of symptoms, but with a sequel: opening of the right labium majus. CONCLUSIONS: Lipschütz ulcer is a rare cause of vulvar ulcer. Its treatment consists of symptom control and it almost always heals within 4 to 6 weeks, leaving no lesions. The diagnosis can only be established after other more frequent causes of vulvar ulceration have been excluded.

ABSTRACT

Abstract The objective of this study was to analyze in the literature how the applications were developed and how they influence the prevention of Sexually Transmitted Infections (STIs). PubMed, CAPES Periodicals and Bireme databases were searched using the descriptors sexually transmitted diseases and mHealth combined by the Boolean connector AND. The search considered studies published between 2013 to 2017 available online in Portuguese and English. ABSTRACT

According to the World Health Organization, each year, more than 376 million people are infected with sexually transmitted diseases, of which 127.2 million are Chlamydia trachomatis, it is a asymptomatic disease, meaning there are no symptoms. Genital infections can lead to complications such as: puerperal endometritis and Fitz-Hugh- -Curtis syndrome. The scientific research was carried out to identify the presence of Chlamydia trachomatis in the endocervical secretion of syphilitic pregnant women. This research established a profile of pregnant women infected with Chlamydia trachomatis using information about possible signs and symptoms during pregnancy and gestational habits. It provides a methodological way to quantify and describe the laboratory research. The results showed, 54% of the pregnant women were diagnosed with Chlamydia trachomatis of which 67% were asymptomatic. We also correlate that high rates of Chlamydia trachomatis infection are associated with lack of knowledge about sexually transmitted infections and their forms of prevention.(AU)

ABSTRACT

ABSTRACT

Objective To determine the association of asymptomatic sexually transmitted infections (STIs) with high-risk human papillomavirus (hrHPV) in cervical squamous intraepithelial lesions and neoplasms. Methods From July 2017 to July 2018, 320 hrHPV-positive and 160 hrHPV-negative women in The First Affiliated Hospital of Xi'an Jiaotong University were divided into normal+low-grade squamous intraepithelial lesion (LSIL) and high-grade squamous intraepithelial lesion (HSIL)+ invasive cervical cancer (ICC) subgroups, respectively, based on their pathological cervical lesion grades. Cervical brush specimens including Ureaplasma urealyticum (UU) and its serotypes, Mycoplasma hominis (MH), Mycoplasma genitalium (MG), Chlamydia trachomatis (CT), and hrHPV were amplified and hybridized using PCR kits (Hybribio Biochemistry Co., Ltd.). The differences between groups were examined by the chi-squared test, continuity correction and Logistic regression. Results The overall infection (at least one of the non-hrHPV pathogens was positive) rates of the two groups were 62.5% and 59.4%, which were not significantly different. The prevalence of UU was the highest. The prevalence of UU serotype 14 (Uup14) and MH was associated with hrHPV (P=0.003, P=0.005, respectively), but not with certain hrHPV genotypes or with single or multiple genotypes. Compared with cases of normal+LSIL, cases of HSIL+ICC had significantly different Uup14 infection rates (OR: 12.579, 95% CI: 3.638-43.497, P<0.001) in the positive group. In hrHPV-negative cases, there were obvious differences in Uuu and Uup1 infections based on different cervical lesion types (OR: 11.646, 95% CI: 1.493-90.850, P=0.019; OR: 7.474, 95% CI: 1.140-49.015, P=0.036). Conclusion Asymptomatic STIs in female lower reproductive tract are widespread. Uup14 may increase the risk of HSIL+ICC when hrHPV is positive. Uuu and Uup1 cause an increased risk of HSIL+ICC without hrHPV.

ABSTRACT

Sexually transmitted infections (STIs) are caused by the spread of pathogens via sexual activity and can cause serious complications if left untreated, regardless of their symptoms. Therefore, early diagnosis of STI is important, and molecular diagnostic methods for rapid detection and monitoring are needed. In this study, we evaluated a multiplex polymerase chain reaction (PCR) kit for simultaneously detecting 13 different bacterial, fungal, and viral microorganisms that cause STIs. The kit performance was evaluated for its sensitivity, lot-to-lot variation, and interference in detecting different pathogens. Additionally, its clinical usefulness was evaluated by estimating its sensitivity and specificity for clinical samples. The limit of detection (LOD) was 0.021–50.104 copies for each pathogen. In the tests of lot-to-lot, 100% of positive samples were detected at low concentrations and negative samples all showed negative results. This result confirms that there is no the variation of lot-to-lot. In the test for interference between pathogens, the efficiency of amplification for each pathogen was not significantly reduced and no nonspecific amplification product was formed. We tested 322 vaginal swab samples using the multiplex PCR kit and confirmed that its clinical sensitivity and specificity were 100% for all pathogens. This multiplex PCR kit can be used widely for rapid diagnosis and monitoring of STIs.
